      2. Certification from the Principal which states that the applicant belongs to upper 20% of the graduating class
      3. Original & Certified True Copy of Form 138 (Report Card): with grades from 1st to 4th grading period

      Note: Students must qualify in the final screening (Qualifying Exam and Interview) administered by the receiving department.

    3. Incoming College Freshmen

      5. Completely accomplished College Application Form
    4. Transferees

      Students who have done college work in other learning institutions and are seeking admission to DHVTSU, for the first time, shall be categorized as transferees. They include those who started college work, transferred to other schools, and are now seeking re-admission. This category also covers applicants to the Graduate Programs.

      1. Honorable Dismissal/Transfer Credentia
      2. Two (2) copies of Transcript of Records – original or authenticated copy
      3. Certificate of Good Moral Character
      4. Original copy of Birth Certificate issued and authenticated by PSA (Marriage Certificate for married women)
      5. Medical Certificate (from the DHVTSU University Physician or other DOH accredited doctors)
      6. Three (3) pcs passport size color picture in white background with nametag
      7. Long white folder
      8. Completely accomplished College Application Form

      Note: Student applicants must not have any outstanding failure in any non-academic subject (including PE & NSTP). In addition, only students who have enrolled in any course leading to a degree program will be admitted as transferees in accordance with the existing policies of the University. Moreover, admission of transferees is subject to availability of slot.

    5. Returning Students
      1. Fully accomplished clearance from the cashier as of the last term enrolled.
      2. Acceptance slip from the Dean of the receiving college or Director of the satellite campus.
    6. Cross-enrollees
      1. Cross-enrolment permit from school or department currently enrolled in.
      2. Certified true copy of evaluation form indicating the subject/s, course code and description of the subject the student wish to cross enroll.
      3. Completely accomplished College Application Form
    7. Shiftees
      1. Certified True copy of grades
      2. Guidance Counselor’s Recommendation
      3. Endorsement from the Office of the Admission
      4. Approved shifting form from the Dean of the receiving collge or director of the satellite campus
      5. Completely accomplished College Application Form

      Note: Students with GWA lower than 85 and subject grades below 80 who wish to shift to another academic program with board exam shall not be allowed.

    8. Foreign Students
      1. Must meet all the prescribed admission requirements of the University and the concerned College where they are seeking for admission.
        • Certificate of Completion of a Secondary Curriculum

        • Original Transcript of Record
        • Affidavit of Support
        • Alien Certificate of Registration (ACR)
        • Certification of Proficiency in English based on TOEFL score (for non-Filipino speaker of English)

        • Security Clearance from the applicant's embassy
        • Resident Guarantor of the applicant's character
        • Photocopy of passport pages where name, photo, birth date and birth place appear
        • Completely accomplished Collge Application Form
      2. Must meet the entire prescribed requirement by the Department of Foreign Affairs (DFA) and Bureau of Immigration (BI)
      3. Foreign students may be admitted based on availability of slots of the course being applied for and are limited to special courses.
    9. Application for Change of Civil Status
      1. Letter on Intent
      2. Original PSA Marriage Certificate
    10. Graduate School Applicants (Master's and Doctorate Program)
      1. Completed application form
      2. Letter of Intent Addressed to Admission Director
      3. Certified True Copy of Transcript of Records
      4. Certification of Grade Point Average from former school
      5. Two (2) Recommendation Letters from superior and/or former professors
      6. Birth certificate issued and authenticated by PSA (Photocopy, bring Original for verification purposes)
      7. Marriage Certificate issued and authenticated by PSA, for married female applicant (Photocopy, bring Original for verification purposes)
      8. Mus pass the UAT
      9. Official Receipt: Testing Fee (Php. 300.00)
    11. For Degree Holder (Second Courser)
      1. Must have complete and valid credentials.
      2. Must meet all the prescribed admission requirements of the University and the college where the applicants are seeking for admission:
        1. Approved Application for Admission for Second Courser /Professional Education Units
        2. Transfer Credentials
        3. Certified True Copy of Transcript of Records  with Special Order Number
        4. Certified True Copy of College Diploma
        5. Photocopy of Birth Certificate issued and authenticated by PSA (bring original copy for verification)
        6. Photocopy of Marriage Certificate issued and authenticated by PSA (for married student)
        7. Certificate of Good Moral Character from former school or Certificate of Good Moral Standing from the employer if employed.
        8. Three pcs Passport size color photo with white background and name tag (taken within the last six months.

Senior High School Admission

Admission Process for Senior High School

  1. 1. Admission:
    1.1.1 The following requirements must be submitted prior to interview and before taking the exam as scheduled by the Guidance Office.
    • Duly accomplished application form;
    • Form 138 (report card) duly signed and certified by the Adviser and Principal;
    • NSO birth certificate (photocopy)
    • Two (2) identical copies of 2’ x 2’ recent photographs, (full-face latest ID) photo with background0 – affix name at the back;
    • Official receipt for testing and application fee amounting to Three Hundred Pesos (P300.00) to be paid at the Cashier’s Office – non- refundable;
    • Must present other evidence as the case maybe, graduation program or the certificate awarded to them.
    1.1.2. Present photocopy and original copy for authentication and reference purposes the following credentials for enrolment:
    The enrollee should be on the list of qualifiers posted by the admission office:
    • Original copy of Form 138 (Grade 10 Report Card or its equivalent);
    • National Statistics Office (NSO) copy of Birth Certificate;
    • Original copy of Certificate of Good Moral Character from the Principal, Guidance Counselor, or Class Adviser;
    • Three (3) pieces of 2” x 2” identification pictures; • Enrollment Application Form;
    • Certificate of Education Subsidy Contract (ESC) indicating the Learning Reference Number (LRN) of Department of Education for those students who are recipient of SHS Voucher Program. (if necessary); and
    • QVR Certificate (if a recipient of the SHS voucher program) 2.1.2 Requirements for Admission for Transferees
    1.1.3 Foreign Students (In coordination with the office of International Affairs)
    The following are required from foreign students intending to study at the university:

      Step 2. Carefully read the information and instructions then fill out the Application Form.
      Step 3. Pay the DHVSUAT Application Fee at the DHVTSU Cashier’s Office; secure the corresponding official receipt.
      Step 4. Submit all requirements directly to the DHVTSU Guidance Center.
      Step 5. Encode Information for Student Profiling at the Office of Admissions.
      Step 6. Secure from the Guidance Center, The “Test Permit,” taking note of the DATE, TIME, and PLACE of examination.
  1. 3. Qualification Process
    1.3.1. For the Academic Tracks such as:
    Science, Technology, Mathematics and Engineering (STEM) Strand
    Accountancy, Business Management (ABM) Strand
    Humanities and Social Sciences (HUMSS) Strand
    General Academic Strand (GAS)

    The following are the qualifications:
    • Must not have a grade lower than 80 in all subjects.
    • Must be of good moral character as certified by the Principal and Guidance Counselor.
    • Must pass the entrance examination and qualifying examination.

    1.3.2 For the Technical Vocational and Livelihood Track (TVL) such as:
    Home Economics Strand
    ICT Strand
    Industrial Arts Strand

    The following are the qualifications:
    • Must not have a failing grade in all subjects.
    • Must be of good moral character as certified by the Principal and Guidance Counselor.
    • Must pass the entrance examination.

College Admission

DHVTSU DOESN'T REQUIRE ANY MINIMUM highshcool average grade requirement for one to take the DHVTSUAT (Don Honorio Ventura Technological State University Admission Test) and apply for any course of the applicant preference. Eligible applicants will just have to comply with all the requirements and pass the DHVSUAT in order to be admitted in the university.

COLLEGE ADMISSION PROCESS

Step 1.
Obtain Application Form through the authorized school representative or directly from the DHVTSU Office of the Admission and Testing.

Step 2.
Carefully read the information and instructions then fill out the Application Form.

Step 3.
Pay the DHVTSUAT Application Fee at the DHVTSU Cashier's Office; secure the corresponding official receipt.

Step 4.
Submit all requirements directly to the DHVTSU Office of Admission and Testing

Freshmen:
     Duly accomplished Application Form
     Photocopy of Form138

Transferees:

Graduate School Admission

Application and Enrollement Procedures for Graduate School Program

Step 1.
Proceed to the DHVTSU Office of Admission and Testing located at the ground floor of the University Library Building (1st Door ). Present all the requirements to secure an Application Form. Carefully read and fill out the form.

Requirements:
     Transcript of Records
     Birth Certificate
     Marriage Contract (if applicable)
     2 pcs. 2x2 ID pictures with name tag(white background)
     Php300.00 (Testing Fee)

Step 2.
Proceed to Window 2 of the Cashier's Office at the Admiistration Building for the payment of the Application Fee (P300.00) abd secure the corresponding Official Receipt.

Step 3.
Submit all the requirements to the DHVTSU Office of Admission and Testing for the Graduate School Admission Test to be administered on the same day. Secure Test Result Slip.

Step 4.
Then proceed to the Office of the Dean of the Graduate School for securing and filling out of the Advising Slip.

Step 5.
Proceed to the Registrar's Office (University Library Building, 3rd Door) for the encoding of the subjects to be take and securing of an assessment form.

Step 6.
Psy tuition fee and other fees at the Cashier's Office

Step 7.
Secure Certification of Enrollement (COE) from the Accounting Office.

Step 8.
Present all pertinent documents to the Registrar's Office to secure class cards.

For OLD Students

        Step 1.
        Present ID and Clearance to the Office of the Dean of the Graduate School to secure Advising Slip.

Step 2.
Fill out the Advising Slip Form and proceed to the Registrar's Office for the encoding of the subjects to be taken and issuance of assessment form.

Step 3.
Pay tuition fee and other fees at the Cashier's Office.

Step 4.
Follow Steps 7 and 8.
