Admission Process for Laboratory High School

Admission Requirements

  1. The applicant must belong to the top 5 of the Graduating Class:
    • Valedictorian
    • Salutatorian
    • 1st Honorable Mention
    • 2nd Honorable Mention
    • 3rd Honorable Mention
  2. Must pass the first and second screening.
    • Entrance Examination
    • Interview
  3. Only fifty (50) students will be taken in.
  4. Fill in the Application form at the Admission Office and bring the following:
    • Two (2) pieces 2x2 pictures (white background)
    • NSO Birth Certificate (Photocopy)
    • Form 138 - Report Card (Original and Photocopy)
    • Certification from the school graduated as honor student
    • Testing fee (PhP. 300.00)
  5. Application Period 
    • March 27 to April 28, 2017

College Admission

DHVTSU DOESN'T REQUIRE ANY MINIMUM highshcool average grade requirement for one to take the DHVTSUAT (Don Honorio Ventura Technological State University Admission Test) and apply for any course of the applicant preference. Eligible applicants will just have to comply with all the requirements and pass the DHVSUAT in order to be admitted in the university.

COLLEGE ADMISSION PROCESS

Step 1.
Obtain Application Form through the authorized school representative or directly from the DHVTSU Office of the Admission and Testing.

Step 2.
Carefully read the information and instructions then fill out the Application Form.

Step 3.
Pay the DHVTSUAT Application Fee at the DHVTSU Cashier's Office; secure the corresponding official receipt.

Step 4.
Submit all requirements directly to the DHVTSU Office of Admission and Testing

Freshmen:
     Duly accomplished Application Form
     Photocopy of Form138
     2 pcs. 2x2 ID picture with name tag (white background)
     Official Receipt
     NCAE Result

Transferees:
     Duly accomplished Application Form
     Photocopy of Grades
     Copy of Honorable Dismissal
     2 pcs. 2x2 ID picture with name tag (white background)
     Official Receipt

Step 5.
Encode Information for Student Profiling at the Office of Admission & Testing

Step 6.
Secure from the Office of Admission and Testing the "Test Permit," taking note of the DATE, TIME and PLACE of examination.

 

Graduate School Admission

Application and Enrollement Procedures for Graduate School Program

Step 1.
Proceed to the DHVTSU Office of Admission and Testing located at the ground floor of the University Library Building (1st Door ). Present all the requirements to secure an Application Form. Carefully read and fill out the form.

Requirements:
     Transcript of Records
     Birth Certificate
     Marriage Contract (if applicable)
     2 pcs. 2x2 ID pictures with name tag(white background)
     Php300.00 (Testing Fee)

Step 2.
Proceed to Window 2 of the Cashier's Office at the Admiistration Building for the payment of the Application Fee (P300.00) abd secure the corresponding Official Receipt.

Step 3.
Submit all the requirements to the DHVTSU Office of Admission and Testing for the Graduate School Admission Test to be administered on the same day. Secure Test Result Slip.

Step 4.
Then proceed to the Office of the Dean of the Graduate School for securing and filling out of the Advising Slip.

Step 5.
Proceed to the Registrar's Office (University Library Building, 3rd Door) for the encoding of the subjects to be take and securing of an assessment form.

Step 6.
Psy tuition fee and other fees at the Cashier's Office

Step 7.
Secure Certification of Enrollement (COE) from the Accounting Office.

Step 8.
Present all pertinent documents to the Registrar's Office to secure class cards.

For OLD Students

        Step 1.
        Present ID and Clearance to the Office of the Dean of the Graduate School to secure Advising Slip.

Step 2.
Fill out the Advising Slip Form and proceed to the Registrar's Office for the encoding of the subjects to be taken and issuance of assessment form.

Step 3.
Pay tuition fee and other fees at the Cashier's Office.

Step 4.
Follow Steps 7 and 8.
